Requirements:
- Required Certifications: MSW, LMSW
- Required Skills: Adult protective services, Advance Directives, Advance Directives, AFDC (Aid to Families with Dependent Children), Documents pertinent information to facilitate care coordination, Medicaid, Medicare, Plan of Care, Power of Attorney, Refers to other disciplines/resources, Short Term Acute Care Inpatient Medical Social Worker, TANF (Temporary Aid for Needy Families), Agency on Aging, Biopsychosocial assessment, Case management, Child protective services, Collaborative care, Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Drug/alcohol use and treatment options, Educates patient/caregiver to facilitate discharge, Facilitate discharge and contingency planning, Food stamps, Guardianship, HCBS (Home and Community Based Services), HMOs, Housing Charities, Knowledge of cultural issues, Long-range planning and decision making, Medical Transportation, Medicare waiver, Medication Assistance, NASW Code of Ethics/Standards of Home Health Care, Prior Authorization, Prior Authorizations, Provide crisis intervention, Refers to rehab and long term care facilities, Review of Health history, Social Security, Veterans Choice Program (VCP)
Job Details:
- Pay: $2240/week - $2432/week
- Assignment Length: 26-week assignment
- Shift: 4, 10-Hour 07:00 - 17:30
- At least one year of experience required
